To request a refond, You'll need to submit a writen request stating the reason for your request and within the first 10 days after the signing of our contract. This can be done by contacting our service department at service@hpvba.com, in person at our office or or by sending a fax. NO refonds of any type will be accepted without a written request.

HPVBA reserve the right to refond based on medical conditions and special circunstances ONLY after a careful evaluation of each case. IF approved, refond will be effective 30 days after we have received the written request by the member.
